Inhalt | Advanced Practice Nurses play a central role in the management of patients with chronic illness especially within the area of self- management support. In this course students will acquire basic clinical competencies to support adult chronically ill patients in view of their self-management. They will be able to asses the patient experience with patient centered communication, to perform tailored assessments, to analyze clinical cases using theoretical background and empirical evidence and will subsequently be able to develop a care management plan for an individual patient with a chronic illness. Moreover, they will position their activities in the broader context of chronic illness management programs. This course thus provides an essential fundament for future courses that integrate several clinical skills, i.e. advanced clinical assessment courses). - epidemiology and impact of chronic illness - lived experience of chronic illness - theory of health behaviour change and associated health behaviour assessment - health behavour change technique and communication - principles and outcomes of models of care for the chronically ill (chronic care models and models for transitional care) - conceptualization and state of art of patient self-management in chronic illness |
Lernziele | The students: 1. critically discuss the epidemiology of chronic illness and its implications for health care delivery 2. describe the state of the art of outcomes research in view of chronic illness management programs 3. reflect critically on the application of state of the art evidence in view of self-management in chronic illness and evaluate the feasibility, processes and outcomes of self-management programs 4. demonstrate skills of patient centered communication and integration of the patients illness experience in the planning chronic care 5. argue on the theoretical underpinning and empirical evidence of behavioural change techniques 6. demonstrate assessment of behavioural and psychosocial factors in a chronically ill patient 7. outline behavioural change techniques in view of identified issue for an individual patient 8. execute behavioural change techniques and communication in view of identified issue for an individual patient 9. explain the characteristics of care models for the chronically ill population in general and and argue on specificities for a particular population |

Hinweise zur Leistungsüberprüfung | the final examinations for this course will take place on 25.05.2016 and 01.06.2016 . A time-planning will be communicated during the Spring Semester of 2016. The final grade is composed by two examinations. 1. Case study (60%) 2. Oral exam (40%) 1. Students will identify a chronically ill patient in their own environment who is willing to be followed up during the semester, to be interviewed, to fill out questionnaires and to participate in some interventional approaches. The patient will give Informed Consent for participating in both courses simultaneously. The patient's condition requiring specific health behaviours should be not too complex. Preferably the patient has one or two chronic illnesses that require a behavior change or investments in health behavior maintenance (i.e. focus should not be on pain or other symptom management issues). The students will write a succinct report on this patient (case study - maximum 3 pages) (60%) in which they report the medical, psychosocial and behavioral issues, analyze relevant factors using theoretical models and empirical evidence, and subsequently develop a care management plan based on the analysis done. This case study will be submitted 2 weeks before the oral exam, and can be written in German or English. 2. The oral exam (40%) will be a based on the case study and the understanding of the student of the course content will be tested using the case study as a basis. |
