Content - epidemiology and impact of chronic illness
- outcomes research in chronic illness management
- multilevel aspects of the epidemiology of and health behaviour in chronic illness
- Conceptualization and state of art of patient self management in chronic illness
- state of the art of health behaviour management (exercise, diet, medication) in chronic illness care
- health behaviour assessment
- Motivational interviewing
Learning objectives This course focuses one of the major challenges for health care in the 21st century: the burden of chronic illness. The epidemiology & burden of chronic illness as well as implications for effective chronic illness care are addressed using lectures, seminars, practical exercises and communication techniques as educational formats.

The students:
- critically discuss the epidemiology of chronic illness and its implications for health care delivery
- analyze examples of chronic illness care in view of their conceptual underpinnings
- describe the state of the art of outcomes research in view of chronic illness management programs
- reflect critically the application of state of the art evidence in view of health behaviour management for various chronically ill populations
- argue the importance of integrating the illness experience of chronically ill patients in the planning of their care
- summarize the evidence in view epidemiology, self management priorities and illness experience for a specific chronically ill patient population
- work collaboratively on a joint project in view of developing a self management program for a specific chronically ill patient population that can guide clinical practice across health care settings
- comprehend the theory and relevance of motivational interviewing for chronic illness management
- carry out a health behaviour assessment and perform a change talk by means of OARS, eliciting change talk, rolling with resistance and finally develop a change plan

Modules Advanced Nursing Practice I (ANP I) (Bachelor Pflegewissenschaft)
Assessment format continuous assessment
Assessment details Students will do a group work (max. 4 person per group). They will summarize the evidence in view epidemiology, self management priorities and illness experience for a specific chronically ill patient population. They will work collaboratively on a joint project in view of developing a self management program for a specific chronically ill patient population that can guide clinical practice across health care settings. They will submit a hard copy of this work by June 25th and give a presentation on June 1st.
70% of grade (20% presentation/50% paper).

MOTIVATIONAL INTERVIEWING
Each student performs a change talk with a chronically ill patient and writes a transcription of this dialogue. Having done so, the student will analyze this dialogue according to the learning goals/Mi-criteria taught during the lectures. This written analysis will be marked with 30%.
